Abstract

The incidence rate of cleft lip and palate in Japan is approximately one in 500 people. In this presentation, I would like to explain the comprehensive team-based medical care for cleft lip and palate treatment at the University of Tokyo Hospital, where I am affiliated. Our department includes oral surgeons, orthodontists, prosthodontists, speech-language pathologists, dental technicians, dental hygienists, and nurses. We work in close collaboration with these diverse professionals to provide comprehensive care for patients with cleft lip and palate. I will now explain the specific treatment procedures.First, prenatal diagnosis may be performed. After birth, lip repair surgery is performed at 3 months of age, and palate repair surgery is performed around 1.5 years of age. Mandibular bone grafting is typically performed before school age or during the eruption of the canine teeth. If necessary, orthodontic Phase I treatment is performed during elementary school. Specifically, if the maxillary canine teeth are likely to resorb the incisors, a windowing and traction procedure is performed on the canine teeth. However, at our hospital, we strive to perform only the minimum necessary treatment. Therefore, the proportion of patients undergoing orthodontic Phase I treatment may be lower than at other hospitals. Orthodontic treatment phase II is performed after the age of 14. In cases of significant jaw disharmony, orthognathic surgery may be employed. Final lip and nose correction surgery is performed after completion of orthodontic treatment. We conduct surveys to gather patient feedback on treatment outcomes and strive to continuously improve patient satisfaction. We collaborate with other facilities in Japan that perform cleft lip and palate treatment through joint studies and seminars to enhance the quality of care provided.
