Title: Advanced implant protocols for managing complex combination defects

Abstract

Rehabilitation of complex combination defects in the anterior maxilla presents a significant clinical challenge due to simultaneous vertical and horizontal bone deficiencies that compromise both esthetics and function. Successful management requires meticulous planning and a prosthetically driven implant approach supported by advanced regenerative strategies.

This report describes the management of a patient presenting with a Class III vertical bone defect in the anterior esthetic zone. A staged regenerative protocol was employed using an autogenous block graft to restore vertical height, supplemented with a xenogenic bone substitute (Zeno graft) to enhance contour and volume stability. Platelet-Rich Fibrin (PRF) was incorporated to promote angiogenesis, accelerate soft tissue healing, and enhance graft integration.

The synergistic use of autogenous bone, biomaterials, and biologics facilitated predictable augmentation of both ridge height and width, enabling ideal implant positioning. This comprehensive approach highlights the role of advanced implant protocols in achieving functional stability and optimal esthetic outcomes in cases involving complex combination defects.
